Member forms are sorted into the following categories:
- Forms used exclusively by a GEPF member while contributing
- Funeral Benefit claims by the GEPF member
- Divorce related forms
- Exit related forms
Divorce related forms
The employer will submit the Z103 to the GEPF, along with the divorce decree (and settlement agreement) If a divorce settlement is payable, the GEPF will inform the ex-spouse and furnish him or her with the choice form for completion.
It is imperative that members inform the GEPF of all divorces as a divorce may affect the value of the member benefit on exit. Where a member has multiple divorces that may affect his or her pension interest, the GEPF must be informed of all the divorces immediately, as the sequence of divorces will impact the calculation of the divorce benefits and reduction of the member service.

Funeral Benefit claims by a GEPF member
A funeral benefit becomes payable on the death of a member, his or her spouse, and his or her eligible child.
The benefit values are outlined in the instructions to the Funeral Benefit Claim Form.
The claimant must complete the Z300 funeral benefit claim form, as well as a Z894 bank form (for a personal bank account at a South African bank, or an overseas banks or estate late bank account).
The requirements for each form are listed in the relevant instruction sheet.
Exit – death of a GEPF member
All the claim forms (Z143, bank forms etc) and supporting documents to be completed by the potential beneficiaries must submitted to the employer as soon as possible. The information on these forms will be added into the Z102 exit form by the employer.
The funeral benefit claim may be submitted directly to the GEPF as per the instructions on the form, or via the employer (submission via the employer will delay payment of the funeral benefit claim).
Where a potential beneficiary is required to submit banking details, the he or she must select the correct bank form based on whether the bank account is a local South African bank, or whether the account is with a bank outside South Africa.
These forms will be submitted to the GEPF by the employer along with the Z102.
Exit – Retirement of a GEPF member
The benefits payable to retiring members differ based on the exit/retirement reason and service period of the member.
Members must confirm the options available and benefits due to them before exiting service, as some exit types such as injury on duty may enjoy enhanced benefits, whereas other exit reasons such as voluntary early retirement may lead to downscaling of benefits.
Where a member is required to submit banking details, the member must select the correct bank form based on whether the bank account is a local South African bank, or whether the account is with a bank outside South Africa.
The forms (and supporting documents) listed under this section must be completed and submitted to the employer as soon as possible. The information on these forms will be added into the Z102 exit form by the employer and the document set will be submitted to the GEPF by the employer.
Exit – Resignation, or retirement/discharge with less than 10 years’ service, of a GEPF member
The benefits payable to members who resign, or are retiring, or who are discharged from service with less than 10 years’ service, differ from fund to fund. These benefits are also dependant on elements such as the exit/retirement reason and service period of the member.
Members must confirm the options available and benefits due to them before exiting service, as some exit types may enjoy enhanced benefits, or the member may be forfeiting the alternative benefits available if the member were to select an alternate exit type (such as resigning when retirement with a monthly pension, is an option).
Where a member is required to submit banking details, the member must select the correct bank form based on whether the bank account is a local South African bank, or whether the account is with a bank outside South Africa.
The forms (and supporting documents) listed under this section must be completed and submitted to the employer as soon as possible. The information on these forms will be added into the Z102 exit form by the employer and the document set will be submitted to the GEPF by the employer.
